I am authorizing my child to participate in Dance Explorers with Explore Dance Company. I understand that there are risks of physical injury associated with, arising out of, and inherent to the activity of dance as well as other physical activity. In recognition of this acknowledged risk of injury, I knowingly and voluntarily waive all right and/or causes of action of any kind, including any and all claims of negligence arising as a result of such activity from which liability could accrue to Explore Dance Company, its officers, instructors, company members, and all affiliated entities including the aforementioned school and PTA. I hereby agree to release Explore Dance Company and hold Explore Dance Company harmless of all liability, and hereby acknowledge that I knowingly and voluntarily assume full responsibility for all risk of physical injury arising out of active participation with/for Explore Dance Company. I am aware that this is a release of liability and an acknowledgement of my voluntary and knowing assumption of the risk of injury. I have signed this document voluntarily and of my own free will in exchange for the privilege of participation.
If any legal action is necessary to enforce the terms of this Agreement, or if it is necessary to employ the services of an attorney to enforce the terms of this Agreements, I agree to pay Explore Dance Company's reasonable attorney’s fees and court costs in addition to any other relief to which it may be entitled if the I fail to pay any amounts owing hereunder when due, or otherwise breaches any terms of this Agreement. I agree to pay up to a 40% collection expense incurred by Explore Dance Company in attempting to collect such amounts in addition to the aforementioned attorney’s fees and costs.
I consent to allow Explore and/or the PTA to provide basic first aid care to my child including but not limited to applying bandaids, ointments, etc.
